Payment
Form Physical Therapy is in network with BCBS, BCN, Aetna, Cofinity, Tricare and Medicare traditional plans. If we do not take your insurance we can still see you as and out of network provider! Some insurance plans will have out-of-network coverage and you may be reimbursed for our services. We have seen up to 80% reimbursement rates with out-of-network coverage. With out-of-network services we do take full payment at time of service from you instead of billing your insurance. We will provide you will a bill to submit to your insurance company after each visit and reimbursement from the insurance company with come directly to you. We are not in contract with all insurance companies so we can afford to provide individual one-to-one quality service at a fair price. Your treatment is provided soley by a Doctor of Physical Therapy and sessions last an hour or more. Health spending accounts (HSA) can be used for these services.

Reschedule/ Cancellation Policy
Please reschedule or cancel at least 24 hours before your scheduled appointment. If we do not receive a request 24 hours before your scheduled appointment time you will be charged a $35 cancellation fee. We often have people waiting to get scheduled and advanced notice of cancellation allows us to schedule someone in the appointment time. It is very important to make your appointments whether it is intended to progress your treatment or just to check in on your progress. We will do our best to meet your scheduling needs.
